Hi

Today we had a member make 9 hits to G143 Pillar (Otis) and it cost him 36 AP instead of the usual 18 AP.

Has this happened before and if so shouldnt this member get 18 AP credited back to his account?

If he lost the PvP phase, then it will be 4 AP per hit. The game used to force you redo the PvP phase until you win, but we changed it so it just cost 2 more AP to proceed into the Pillar fight when you lose few updates ago. He might not have paid attention and assumed he won the PvP phase.
