We did find a bug last week that would prevent it from triggering. Since it will take too long to properly fix the bug with an update due to Apple's current extremely long approval time, we temporarily fixed this on the server by changing the behavior of the passive skill to give a small chance to deal 50% more damage with normal attacks (which is actually much better than doing additional weaker normal attack). The passive skill description/name will be changed to match this behavior on version 1.13 (version 1.12 have already been sitting in the review queue for 9 days now).
